I know that this is stupid question and that I should get one from some official institution but I never got it.

Now I need this document but I do not know where I can order it. Does anybody has some phone number or any information about it ?

You should ask for it at your Medical-Insurance, in my case it is "Techniker Krankenkasse". They told me that the Ausweis itself will be issued by Pension Fund (BfA), but it is requested through Krankenversicherung (?). The TK sent me the confirmation that I have requested the Sozialversicherungausweis. All of this happened in May 2003. I however did not get the Sozialversicherungausweis until now :-))
