Hi,

Could you please tell me what shall be the content of this Notarised
Affidavit?

When I talked to the Indian consulate, they say that I just need to put the
ad. and attach that copy along with the name change application and then I
sign the Affidavit in front of the Counsel General for changing the name-
not before that.

I also talked to one of my friend who published ad. for name change in
Germany through Ads2book, told me that you did not ask for Gazette
notification or any such thing.

I have attached the extract from Indian consulate, Frankfurt website that
describes about the process for name change. Please refer section 4 in the
attached pdf. It says affidavit has to be signed in the presence of consular
officer as final step of name change. For me this looks like a chicken-egg
problem.

Please clarify.

Thank you

Regards
Kumaran

Dear Sir,
As per company policy and legal guidelines we require a Notarised Affidavit
for the publication of the same. In case you are based outside India, we
require a Notarised Affidavit Attested by the Consulate General of India /
Indian Embassy. The ad cannot be published in any of the Publications unless
we receive the required documentation mentioned above by e-mail (as
attachment) post, 5 days prior to publishing date. The document must also
include father's name & address (same as to be published in the ad text).

Regards,
Saba
