Anybody have recently applied for work permit from Pakistan?

I have job offer letter/contract from a German employer and planning to apply for work visa (blue card). I am looking for the following information:

- What documents do I need to submit at embassy?

- I am wondering do I need language certificate, accommodation and health-insurance details for applying the visa.
