Hello,
First of all is she a german national or a national of your country. For the former you neet to go the Einwohneramt and request for a new Ausweis. And other formalities will be done automatically.

For the later, here are the formalities.

1)First you have to go to the embassy of your country, show the necessary documents(Passport, Marriage Certificate, Residential Proof). You will be issued a new passport.

2) Take this and the old one to the Auslanderamt and inform them about the change. They will stamp a visa(copy of the old one) in her new passport. You need to pay 10€ for that.

3) If she is working then go to the arbeitsamt to do the rest of the formalities.

Hope this helps you. For further details just contact the embassy of your country.
