What are the requirements to reenter in U.S. after traveling abroad?

Hi Everyone,
As per my information, following are the requirements:
  1. You should have been out of the U.S. for less than five months.
  2. Your passport is valid for at least six months only after the date of your reentry.
Is this correct information ? IF you have any advice please send me.
