Visa Form Question

Hi all,

My wife is applying for the spouse for Germany. In the visa form, there is a question which says "Have you ever been expelled or deported from the Federal Republic of Germany, had an application for a residence permit rejected, or been refused entry into the Federal Republic of Germany?"
My wife's visa for Germany has been refused once. It was a tourist visa application. After that, she again applied for the Germany tourist visa and came to Germany for 1 month.
Now my question is : For the question above, should I write "Yes" or "No". If yes, then should we mention the details as well?
