Number of travel Document : Your passport number
 Residence permit No.  : You can keep it blank or fill your spouse's residence permit number. In the later case, you need to mention explicitly that the residence permit belongs to your spouse.
Valid until  : keep it blank or  fill from your spouse's residence permit (again mention that it is your spouse's residence permit.)
